Immunization Requirements 2015-2016

As you prepare for the 2015-2016 school year, please ensure that your child’s immunization record is updated and that documentation is submitted to Unity by  8/30/2015.  Below is a list of the immunizations that your child needs for each grade. If your child is turning 11 at some point during this school year, please schedule to have her/him vaccinated on her/his birthday or the day after.  Students who do not have all of the required immunizations will not be allowed to enter the school building until they have received all the required immunizations. Additionally, all students must complete a thorough physical for the year. Please complete the Health Examination Form and return it to Unity if there is not a physical form on file already.

VACCINES Grades 6 & 7                Grade 8            
Diphtheria and Tetanus toxoid-containing vaccine(Dtap/DPT/Tdap) 3 doses 3 doses
Tetanus and Diphtheria toxoid-containing vaccine and Pertussis vaccine booster(Tdap) 1 dose at age 11 1 dose
Polio (PIV/OPV) 3-4 doses 3 doses
Measles, Mumps, and Rubella (MMR) 2 doses 2 doses
Hepatitis B 3 doses 3 dose
Varicella (Chickenpox) 2 doses 1 dose

Summer Academy Begins July 6th

Please note that students who are attending Summer Academy will be with us from July 6th until July 30th.  Please contact your child’s advisor for more information.

Friday, May 1st – No school for students

Students will not have school this Friday, May 1st.

Metro Card Increase and NYC Survey

The NYC DOE would like parents to take part in a survey that will help to “plan the city’s future.”  The survey can be found here.

The MTA has recently increased the price of metro card fares.  This increase will change“half-fare” metro cards in which the student is required to pay the other half of the fare.  The fare has raised from $1.25 to $1.35 per trip.

National History Day Competition!

Unity Prep was well-represented yesterday at the 25th Annual NYC National History Day Competition!  All four groups presented their documentaries or exhibits with pride, and we heard a lot of plans for next year’s competition based on inspiration from the other projects at the event!  On top of that, Michaiah, Nathanial, and Antonio won the award for Outstanding Project from Brooklyn!  It was great to see our kids in the spotlight.
 
Thanks to Mr. Boolani for inspiring and leading this whole project, Ms. Augustine and Ms. Moor for being amazing chaperones and supporters, Mr. V. and Ms. Yang for coming to see the event, and, of course, Shaena, Rama, Daniella, Kimron, Jemuel, Nicholas, Ronerdy, Sheiqkel, Michaiah, Nathanial, and Antonio for being heroic historians!

Early Closing Due to Snow 03/05/2015

Please note that we will dismiss at 3:15 today.  Clubs, study hall, test prep, and the club showcase are canceled.

School Closed 02/16, No Saturday Tutoring or Mentoring

Unity will be closed on 02/16 to observe President’s Day.  Saturday tutoring and mentoring will not meet this Saturday, 02/14.

School is closed 01/27 due to weather
